To be eligible for this rebate you must have a Chronic Disease Management (CDM) Care Plan from your GP. A copy of this plan, EPC form and a GP referral letters is required to confirm the appointment. Gap payment apply and depending on the consultation type. As of July 2024, Medicare Rebate for CDM is $60.35.
